The Registered Nurse is responsible for the skilled care and treatment of the Health Care Partners, Inc. patient following THE AMERICAN NURSES ASSOCIATION'S STANDARDS OF PRACTICE. The nurse functions as Case Manager in order to facilitate coordination of care among all disciplines and implement the physician's plan of care.
The Occupational Therapist is responsible for the evaluation and treatment of the functional, motor, perceptual-motor, sensory, and cognitive deficits of Health Care Partners, Inc. patient under the physician's direction. The therapist utilizes specialized restorative techniques in accordance with the American Occupational Therapy Association guidelines to improve our patient's level of independent, safe function in activities of daily living. The therapist may act as Case Manager and supervise the Certified Occupational Therapy Assistant and the Home Health Aide as necessary.
The certified Home Health Aide is responsible to provide personal care services to Health Care Partners, Inc. patient under the direction of the Case Manager. The Certified Home Health Aide will follow a plan of care developed from the physician's treatment plan supporting independence in the performance of activities of daily living. The Home Health Aide will follow the guidelines of the Foundation for Hospice and Home Care.
The Medical Social Worker is responsible for the assessment of social and emotional factors impacting the Health Care Partners, Inc. patient's health. The social worker will provide community resource planning, counseling services and assessment of need for alternative living arrangement. The social worker will utilize short-term therapy for coping with medical conditions, chronic disease disability or terminal illness in accordance with state guidelines as a Certified Social Worker (CSW)
The Speech Language Pathologist is responsible for the skilled evaluation and the treatment of speech, swallowing and cognitive deficits of Health Care Partners, Inc. patient under the direction of the physician. The therapist utilizes specialized therapeutic techniques in accordance with the American Speech and Hearing Association guidelines. The therapist supervises the Home Health Aide as necessary when acting as a Case Manager
The Physical Therapist is responsible for the skilled treatment of neuromuscular and musculosketetal disorders of Health Care Partners, Inc. patient under the direction of the physician. The therapist provides therapeutic rehabilitation in accordance with the American Physical Therapy Association guidelines. The therapist acts as Case Manager when necessary and supervises the Physical Therapy Assistant and Home Health Aide.
The Registered Dietician serves as a consultant to Case Managers for developing nutritional care planning for Health Care Partners, Inc. patient under the physicians direction. The dietician will utilize nutrition education in working with staff, patients and caregivers as well as meeting community needs. The dietician works under the guidelines set forth by the American Dietetic Association.

emotional, spiritual and ethical concerns.
The Diabetes Self-Management Program is coordinated by a Certified Diabetes Educator who supervises our team of instructors. The instructors are qualified Registered Nurses and Registered Dieticians. Our Diabetes Self-Management Program has been awarded Education Recognition by the American Diabetes Association.
